Resolution 09-06 RECOMMENDING THE
CREATION OF THE CONTINUUM OF CARE PLANNING COMMITTEE TO CONTINUE THE REDESIGN
OF LONG TERM HEALTH CARE TO INCLUDE THE DEVELOPMENT OF A FACILITY IN SAUK
COUNTY
- " . . . that a Continuum of Care Planning Committee be formed
to continue developing strategies to address gaps identified within the
continuum by working with existing oversight committees and departments;
- "incorporate citizen and staff participation as appropriate as
they formulate recommendations;
- proceed with the requisite planning for the development of the continuum
of long term care through the construction of a facility
that will incorporate a skilled nursing function and may provide for:
Dementia beds; respite / crisis beds; adult day care; county prepared
home delivered meals; home based therapies; assisted living; affordable
housing; or other services as appropriate;
- study the services that should be provided within this facility, develop
recommendations regarding appropriate business models for
the facility to incorporate best practices, and make recommendations
for design;
- conduct an operational analysis of the existing facility to identify
changes to the business model consistent with the realignment of service
provision;
- that the Continuum of Care Planning Committee incorporate evaluation
design as an essential component of program design;
- that the Continuum of Care Planning Committee report
their recommendations concerning services to be provided,
potential site, number of beds, and other related information
to the Board of Supervisors by December, 2006.
Goals (draft from flip chart)
- It is intended that this facility will not compete with other nursing
homes within Sauk County, but provide serves those with
behavioral and medical issues that cannot be dealt with in
other venues. It is intended that this facility will fill a gap for those
with no other resource, as well as being an enhancement to the options
available to Human Services. County services are not intended to replace
other services offered within the county.
- Any practice best handled by privates should be handled there.
- Right capacity for the market to be served, balancing the concern over
excess capacity and competition for beds.
- Efforts consistent with state plans
- Consider stream of services for elderly and disabled for an integrated
and coordinated service model, understanding that knowing
what to provide is as important as knowing what not to provide.
- Balancing service with cost to minimize impact to Sauk County taxpayers.
Goals (from previous committee work)
- Fostering and providing an integrated seamless service model that spans
the continuum.
- Enhancing individual functioning and quality of life.
- Fostering and providing programs consistent with meeting targeted
needs.
- Meeting all federal and state mandates.
- Creating effective and efficient programing that is flexible,
responsive, fiscally responsible and sustainable.
- Identifying areas for enhanced programming, and maximization
of resources.
- Development of social capital. Social capital is defined as "the resources
available to individuals and groups through their social connections to
communities" (Cannuscio, Block, & Kawachi, p. 395). It is access
to this social capital which enables people to live "productive, independent
and fulfilling lives" (p. 395). Service models that are integrated
with vital community resources hold the most promise for
creating social capital.
