Continuum of Care Planning



The regular meeting of the Sauk County Continuum of Care Planning Committee was called to order and certification of Open Meeting compliance was given at 6:00 p.m. by Joan Fordham.

Members present: Paul Endres, Linda Borleske, Al Dippel, Lowell Haugen, Joan Fordham

Members excused: Judy Stoeckman, Scott Alexander

Others present: Kathy Schauf, Ken West, Tony Tyzchinski, Cindy Bodendien, Sue Hebel, Media

The agenda was approved on a motion by Borleske, second by Dippel, and a unanimous vote of the Committee. The minutes of the prior meeting were approved on a motion by Endres, second by Haugen, and a unanimous vote of the Committee.

Endres commented on diligence of staff at the Health Care Center over weekend storm.

Business Items:

Health Care Center Building Project

Discussion of meeting schedules. Joint meetings scheduled as needed. Contract issues will be developed and discussed.

Aging and Disability Resrource Center update

Organizational chart, governance, and location options were discussed. Initiatives at the state and learning from other counties were discussed.

Site visits.

Information collected will be used for design and communication with the architect.

Consolidation of significant issues from Jefferson County regarding things to avoid etc. From site visit on January 30, 2007 will be provided by commitee members.

Vision and Mission of A27ging and Disability Resource Center

New vision and mission were distributed, along with information of mandatory ADRC services. Motion by Borleske, second by Dippel to have "By fulfilling our mission" sticken from vision statement, Motion carried.

Group will continue to consider goals and outcome and efficiency measures.

Timeline and task list

Reviewed for accuracy and a check of activities.

Next meeting date was set for 6:00 p.m. on March 13, 2007.

Meeting adjourned on a motion by Borleske, seconded by Dippel, with a unanimous committee vote.
