Call to Order and Certification of the Open Meeting Law: Carlson called the meeting to order and certified compliance with the open meeting law.
Adopt Agenda: Irwin moved to approve agenda, seconded by Hill, passed by unanimous vote.
Approval of June 6, 2007 meeting minutes. __________ moved to approve minutes, seconded by __________, passed by unanimous vote.
COP Exceptional Expense Request: Hill moved to approve the COP Exceptional Expense request, passed by unanimous vote.
COP Policy 1.67: Discussion regarding 1.67 referrals. Reviewed section 2.10 of the Waiver Manual and the Community Options Waiting List Policy. Reviewed the draft policy dated October 17, 2007, regarding the use of COP funding for Interdivisional Agreement 1.67 eligible person. Draft policy approved and motion passed by unanimous vote. See attachment.
Sauk County Health Care Center Continuum of Care Update: Joan Fordham, Chairperson of the Continuum of Care Committee, provided history and updates regarding the Sauk County Continuum of Care Committee. Ms. Fordham explained the role of the committee. The committee made the decision to recommend that the county continue to operate a nursing home. The committee recommended that a new nursing home facility be built and recommended it be built on a site located in Reedsburg near the MATC campus. Ms. Fordham explained, with the plans for LTC redesign in Wisconsin which will expand community care for the elderly/disabled individuals, the new nursing home will be downsized to an 82 bed facility. Tentative plans for groundbreaking will be in the Spring of 2008. The Sauk County Health Care Center is in the process of hiring a new administrator.
Also, Ms. Fordham explained that Sauk County joined a coalition with LaCrosse County enabling Sauk County to place residents who have dementia/significant behavioral needs at a LaCrosse County facility to receive care and supervision.
Long Term Care Redesign Update: The ADRC positions were approved by the Sauk County Board on October 16, 2007. Positions will not be hired until the State budget has been passed. Regions originally scheduled to start in January 2008 have pushed back start dates three months. Several workgroups continue to meet. Sauk County continues to plan with hopes of an ADRC and CMO opening by the end of 2008.
Committee Membership: Discussion regarding Alice Moyer and Theron Hill terms expiring at the end of April 2008. Theron and Alice agreed to serve another term. The committee approved and will recommend continued membership of Theron Hill and Alice Moyer.
Training Opportunities: Thill passed around upcoming training events from the Bureau of Aging and Disability Resources. Thill reminded committee members that they're required to attend one training per year.

Policy: COP FUNDING FOR AN INTERDIVISIONAL AGREEMENT 1.67 ELIGIBLE PERSON
A person, who is referred through Interdivisional Agreement 1.67 under s.46.27 (6r) (b) (3), is eligible for COP. A Medicaid Waiver participant whose program services have been terminated because of an ineligible level of care is eligible for COP as an I.A. 1.67.
Sauk County Department of Human Services (SCDHS) will serve I.A. 1.67 individuals with COP when COP funding is available. If COP funds are not available at the time of the referral, the person will be placed on the SCDHS Community Options waiting list. The person will be served from the waiting list in accordance with the SCDHS Community Options Waiting List Policy dated November 16, 2005, revised 02/2007.
When an I.A. 1.67 COP eligible person is served, he/she will receive COP funds for one year from the initial COP start date. At the person's annual recertification, if the level of care has not changed, and the individual is not Waiver eligible, COP funding will be terminated.
