Using grant funding, the Sauk County Health Department is establishing overdose response teams. These are teams of 3 individuals (2 EMS/Fire representatives and 1 peer support specialist) that would attempt an in-home or in-facility visit to overdose survivors in Sauk County in an effort to streamline
a connection to treatment and other vital support services. The BAA attached is a template used by Milwaukee Rapid Response Teams. Its purpose is to enable the sharing of protected health information between each participating agency (EMS/Fire and peer support agencies) and Sauk County Public Health so that 1) public health will be alerted when an EMS provider treats a suspected overdose and the name and contact information of the person who overdosed will be provided to public health; 2) public health will then share the name and contact information of the individual with the team members who have been scheduled to attempt the home/facility visit.
