The Sauk County ADRC Nutrition Program is a proud member of Meals on Wheels America!

Check out one of our 5 dining centers in Sauk County! Your community’s Dining Center holds regular activities and hosts guest speakers to help you stay healthy and “plugged in” to your community. Dining Centers create a great opportunity to socialize and stay independent.  Please make  reservations for lunch 24 hours in advance. For more information on diner eligibility see info below.

Our My Meal, My Way Restaurant partnerships still run on a contribution basis. Choose Your Meal, Your Way!

Diner Eligibility

Individuals eligible to receive a meal on a voluntary contribution basis at any of the ADRC dining sites are: aged 60 or older, a spouse/domestic partner who eats a meal on the same day as a spouse/domestic partner who is aged 60 or older, and an adult with a disability younger than 60 who resides with an eligible older individual. Individuals who do not meet the above criteria will kindly be asked to pay the full cost of the meal.

Nutrition Education

Each month, Home Delivered Meal participants and Congregate Diners are given nutrition education in the form of a flyer. This flyer is available below under "Supporting Documents".

Meals on Wheels

Meals on Meals are also provided to the homebound.  If you are interested in learning more, call the Aging & Disability Resource Center office at (608)355-3289 to inquire about service in your area.  Your medical provider may also make a referral for this service if you have been hospitalized or have been ill.

Depending on where they live, participants will receive a home-delivered meal each day OR frozen meals delivered once a week.  A volunteer will arrive between 11:00 am and 1:00 pm with lunch. (All lunches include 1/3 of recommended dietary allowances as suggested by the United States Department of Agriculture's "My Plate" guidelines.)

FoodShare

If you have a moderate income, especially if you have high medical expenses, you may be eligible for FoodShare. FoodShare provides recipients with a card that is reloaded with funds each month, providing the recipient with the means to purchase grocery items from stores such as Pic n Save, Viking, Walmart, etc. For more information on FoodShare or information on how to apply, click here. For help filling out an application for FoodShare, call Sauk County Human Services at (608) 355-4200.
