Contract between ADRC and AAA Older American's Act Programs Title III- Coronavirus Aid, Relief, and Economic Security (CARES) Act Additional Funding

This amends the original 2020 OAA Contract agreement for the period of January 1, 2020 – December 31, 2020 by and between the "GRANTOR" Greater Wisconsin Agency on Aging Resources (GWAAR), Inc., and the "GRANTEE", ADRC of Sauk County.

This amendment will serve as notification that the GRANTOR is unilaterally changing the amount of the grant by the amounts shown in the table below, in column "Contract Change Amount" and making the total payment to the GRANTEE in the amounts also shown in the table below, in column "New Contract Amount"

All other terms and conditions of the original agreement remain unchanged.

GRANTOR and the GRANTEE acknowledge that they have read and understands this Amendment and agree to be bound by the amendment’s terms and conditions. GRANTOR reserves the right to reject or cancel amendments based on documents that have been altered. This amendment becomes null and void if the time between the earlier dated signature and the later dated signature exceeds sixty (60) days, unless waived by the Department of Health Services (DHS).
