Contract between ADRC and State of Wisconsin Department of Transportation- Section 5310 Enhanced Mobility for Seniors ad Individuals with Disabilities

This Grant Agreement is made by and between the State of Wisconsin Department of Transportation ("the Department") and County of Sauk ("the Recipient").

The Department agrees to provide financial assistance with program monies made available in accordance with the terms and conditions of this Grant Agreement and the provisions of the Recipient's 2020 Section 5310 Enhanced Mobility for Seniors and Individuals with Disabilities application for funding assistance, which is made part of this Grant Agreement by reference.
