Contract between ADRC of Eagle Country Office and WI Department of Health services - SHIP.

SHIP grant funding is provided by the Wisconsin Department of Health Services (DHS) under

Grant Number 90SAPG0091-01-00 from the U.S. Department of Health and Human Services -

Administration for Community Living. The current grant period begins April 1, 2020, and ends

March 31, 2021.

The purpose of SHIP grant funding is to provide information, counseling and assistance that will

help Medicare beneficiaries to understand and access Medicare and related program benefits.

NOTE FOR ADRC: On profile 560432, GRANTEE is receiving $5,400 SHIP funding for supporting Crawford

county, $3,500 SHIP funding for supporting Juneau county, $5,970 SHIP funding for supporting Richland

county, and $4,330 SHIP funding for supporting Sauk county.
