This SERVICES AGREEMENT (“Agreement”) is effective __(last date of signature)___________, 2023 (“Effective Date”), by and between Sauk County Health Department (“PARTY”) and SSM Health Care of Wisconsin, Inc., a Missouri nonprofit corporation, doing business as SSM Health St. Clare Hospital - Baraboo (“SSM”).
