SIGEND FINAL Flourish & Thrive Labs BAA 6-3-2024

This Business Associate Agreement (the “BAA”) is made and entered into as of June 1, 2024 (the "Effective Date") by and between Flourish & Thrive Labs, LLC, an Illinois limited liability company ("Business Associate", in accordance with the meaning given to those terms at 45 CFR §164.501) with an office located at 140 Mallard Court, Grayslake, IL 60030, and Public Health Sauk County, a local public health department in Wisconsin, with an office located at 505 Broadway St #372, Baraboo, WI 53913 ("Covered Entity"). In this BAA, the Covered Entity and Business Associate are each a “Party” and, collectively, are the “Parties”.
