YRBS Data Use Agreement

This data use agreement is between the Health Department and individual school districts in Sauk County. The Health Department would like to create a Sauk County aggregate data set for the high school level Youth Risk Behavior Survey which was conducted in 2019 in 3 Sauk County high schools. These data will allow our department and community partners to assess youth behaviors in order to prioritize, implement, and evaluate interventions that can improve the lives of Sauk County youth.
