Fatal Traffic Crash on USH 12/CTH PF

On May 14th, 2019 at 5:45pm the Sauk Prairie Police Department, the Sauk City Fire Department and Sauk Prairie EMS were dispatched to a two vehicle crash at the intersection of  HWY 12 and County HWY PF in the Village of Prairie du Sac.  Initial investigation and observation shows a white Chevy sedan traveling southbound on HWY 12 turned east onto CTY HWY PF in the path of a north bound Chevy SUV.  The SUV struck the sedan on the passenger side. The passenger of the sedan was extricated from the vehicle and pronounced dead at the scene.  The driver of the Chevy sedan was transported to UW Hospital with serious injuries.  The driver of the Chevy SUV was transported to the Sauk Prairie Hospital with serious, but not life threatening injuries.  All parties involved were wearing seatbelts at the time of the crash, and speed does not appear to be a factor.  As a result of the crash, CTY HWY PF was shut down to traffic and HWY 12 was restricted during the investigation.  All traffic lanes were reopened to normal traffic patterns at 8:05pm.  Names of those involved are being withheld at this time until notification of family can be made.

The crash remains under investigation by the Sauk Prairie Police Department and the Sauk County Sheriff’s Department reconstruction team.  Also assisting in the investigation was the Prairie Fire Department, Sauk Prairie EMS, Baraboo EMS and paramedic unit and the Sauk County Coroner’s Office.  Anyone with information regarding this crash is encouraged to contact the Sauk Prairie Police Department at 608-643-2427 or 608-355-4495.
