NEW Diner Form

Find the ADRC NEW Diner Form below. Print, fill out, and bring with you the first time you pick up a meal. The information on the form is for data reporting purposes only and will be kept secured and confidential. 

Eligibility: adults 60 and older are eligible for a meal at a suggested donation rate of $4.00. A spouse who is eating with an eligible diner on the same day is also eligible for the donation-based meal.
