Informal Administration

A Personal Representative is nominated by the testator in a Will.  If there is no Will, interested parties to identify and consent to appointment of a Personal Representative to act.  A Personal Representative shall not act in that capacity until issued Domiciliary Letters by the court.

To Open an Estate:

  Original Will

  PR-1801 - Application for Informal Administration

  PR-1806 - Proof of Heirship

  PR-1803 - Waiver and Consent - for nominated personal representative and each heir/beneficiary

  PR-1808 - Statement of Informal Administration

  PR-1804 - Notice to Creditors

  PR-1807 - Consent to Serve as Personal Representative

  PR-1810 - Domicillary Letters

  Proof of Publication - received 1 month after first publication of Notice to Creditors - filed with court.

  PR-1811 - General Inventory - decedent's assets as of date of death - filed no later than 6 months from the date the Personal Representative was appointed.   Filing fee of .2% ($20.00 minimum) of net    value of inventory.

PR-1817 - Affidavit of Mailing - proof of Inventory being provided to interested persons.

PR-1814 - Estate Account - final accounting (with schedules) of estate from date of death to closing.

If added property under Schedule A include additional filing fee of .2% x total amount of added property.

PR-1817 - Affidavit of Mailing - proof of Estate Account being provided to interested parties.

PR-1815 - Estate Receipt - received from creditors when claim is paid; from beneficiaries for partial and full distributions.

Closing Certificate for Fiduciaries - obtained from Wisconsin Dept. of Revenue after fiduciary tax return with request for Closing Certificate filed with DOR.

PR-1816 - Personal Representative's Statement to Close Estate - due at completion of estate.

 

Claims Against the Estate:

Personal Representative or Attorney must verify whether or not claims have been filed by either of two methods listed below:

Contact Register in Probate Office (please provide case number) to determine whether claims have been filed.

Search the case on the internet at:  www.wicourts.gov under case search - provide case number and county or name and county.   Note:  all claims to be satisfied before estate can be closed with Estate Receipt or release filed for each claim.

  Certified Copies:

If certified copies are required (financial institution, real estate agency, title company, etc.), the cost is $3.00 for the certification and $1.00 per page.
