Who we are:

The Community Support Program (CSP) is a treatment program for persons with serious and persistent mental illness. A range of treatment, rehabilitation and support services are provided through a team approach. The Sauk County CSP team includes; a psychiatrist, a physician assistant, psychiatric nurses, social workers, psychotherapists and mental health technicians. The Sauk County CSP office is located in Reedsburg, WI but services are generally provided in the community throughout Sauk County.

What we do:

The purpose of CSP is to focus on recovery through providing well-rounded, community-based care in the least restrictive environment possible. Services include:

  • Psychiatric evaluation and assessment
  • Professional assistance with medication management
  • Case Management and Psychosocial Rehabilitation
  • Identifying work related interests or skills, help navigating employment resources, or help developing interests that could grow into possible employment.
  • Connecting to community resources that allow consumers to increase their independence.

Each CSP consumer has a designated case manager who oversees their care and coordinates any needed services. The case manager collaborates with the consumer to develop a recovery plan, which includes short and long-term goals. Frequency of contact can vary and is determined by level of need. Symptoms and treatment progress are continually monitored and services are adapted to meet those changing needs. CSP staff are highly trained to recognize changes in symptoms and work closely with the consumer to help them maintain safety and stability in the community.

How to Access:

Admission to the CSP program is determined by the admission criteria defined in DHS 63 of the Wisconsin State Statutes. In general, an individual must be diagnosed with a serious and persistent mental illness and be at risk of continuing in a pattern of institutionalization or living in a severely dysfunctional way.

The Community Support Program is considered a high level of care. Sauk County Human Services also offers other Mental Health Services that may be less intensive through Mental Health and Recovery Services and the Comprehensive Community Services Program.

To request mental health services through the Sauk County Human Services Department, call (608) 355-4200 to speak with an access worker. The access worker will assist with referring you to the appropriate program in order to start the screening and assessment process.

Cost:  You will not be charged for Community Support Program services.  However, we are reimbursed for some services by Medicaid or insurance companies.  You may be responsible for the cost of your medications.
