Community Health Needs Assessment

We want to hear from you!

Sauk County Health Department along with our partners Reedsburg Area Medical Center, SSM Health - St. Clare Hospital, and Sauk Prairie Healthcare are launching a joint assessment of the health of our communities.  We are working together to assist in determining where our positive health efforts should be focused. There will be several opportunities for community members to participate with us in this assessment. The first step is that all participating partners will post a survey to complete on their websites, Facebook, and Twitter accounts.

 It can also be accessed at https://www.surveymonkey.com/r/saukhealth.

The survey will be available from April 4th through July 16th. It only takes about 10 minutes of your time to complete.

You will be eligible to win an amazing prize for assisting us by completing the survey.

The Health Department will also be holding listening session throughout the county in the coming months to get your feedback. Please contact the Health Department at 608-355-3290 if you are interested in attending a listening session.

Working together, we can ensure that Sauk County is a healthy and happy community.
