Creating a Healthier Sauk County—Together
Our Vision: Health for All

Everyone deserves to live in a healthy, thriving community. But how do we know what needs to change—and how to make it happen?

Every three years, Public Health Sauk County works with local partners to take a deep look at the health of our community. This is called a Community Health Assessment, and it's how we find out what people need to live healthier lives.

We team up with organizations like the Sauk County Data Council, Reedsburg Area Medical Center, Sauk Prairie Healthcare, SSM Health St. Clare Hospital, and the Ho-Chunk Nation. Together, we collect input through surveys, focus groups, interviews, and data. Then, we bring the community together to share what we’ve learned and hear your ideas.

  • Housing
  • Substance Use

To tackle these issues, we lead community action teams that meet regularly. These groups are open to everyone—yes, you too! We dig into the root causes of these challenges and work on real, lasting solutions that can make a difference.
